Forestplot - Error in prFpConvertMultidimArray(mean) : Sorry did not manage to correctly identify the upper/lower boundaries from the input matrix.

I am fairly new to the R programming and community, so apologies if this is a topic that has come up many times before.

I am trying to generate a forestplot and cannot get it to work, I keep getting the error message:
Sorry did not manage to correctly identify the upper/lower boundaries from the input matrix.

My code looks the following:
