pivot_wider question

Hello all ,

What I doing is trying to get various visits and it's dates
what I need

Usubjid visit_1 visit_2 visit_3 visit_4 visit_5
100 2013-01-11 2013-12-12 2013-12-30 2014-01-30 2014-02-10

when running this code
sv_head <- head(sv)
sv_t <- pivot_wider (sv_head , id_cols = usubjid ,
names_prefix = "visit_" ,
names_from = visitnum ,
values_from = svstdtc )

but when I run the above code on sv I am getting bunch of numerical values as dates
like
Usubjid visit_1 visit_2 visit_3 visit_4 visit_5
100 512 517 537 549 551

Also giving me warning messages with values_fn
